SALMON VNA serves individuals in more than 40 cities and towns in Central Massachusetts, and provides a broad variety of assistance including skilled nursing; maternal and child health nursing; mental health nursing; rehabilitation services; medical social services; and home health aides. Their specialty service of Telehealth allows home monitoring for quick response to changes in a person’s health, and their fall prevention program creates care plans to enhance safety. SALMON Health and Retirement’s Milford Campus also features the offices supporting SALMON Hospice as well as SALMON Private Care Options. All of these services are delivered in the field; i.e., in people’s homes, wherever they may live.   • Evaluate residents to assess physical dysfunction and set reasonable goals for progress in cooperation with the healthcare team
  • Develop evidence based treatment programs with ongoing assessment and modification pending patient response
  • Administer, supervise and schedule direct resident care while utilizing appropriate physical therapy techniques
  • Collaborate/delegate/supervise assistant staff and therapy extenders
  • Maintain resident records pertinent to their care
  • Provide appropriate referrals to other healthcare providers as needed
  • Understand self-need for independent and ongoing professional growth and development as PT



  • Graduate of an accredited Physical Therapy with valid MA licensure
  • Must be willing to travel throughout the Central Mass service area
  • Weekend availability a must
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, families, personnel and support agencies
  • Strong written and verbal communication and effective interpersonal skills, with the ability to manage multidisciplinary projects
